Job Title/Position: Certified Home Health Aide

JOB DESCRIPTION SUMMARY
The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting Jhese observations and documenting observations and care performed.
The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient’s care. ·
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Responsibilities of the home health aide include, but are not limited to, the following:

1. Providing personal care including:
   1. Baths
   2. Back rubs
   3. Oral hygiene
   4. Shampoos
   5. Changing bed linen
   6. Assisting patients with dressing and undressing
   7. Skin care to prevent breakdown
   8. Assisting the patient with toileting activities
   9. Keeping patient’s living area clean and orderly, as appropriate
2. Planning and preparing nutritious meals.
3. Assisting in feeding the patient, if necessary.
4. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and l;>lood pressure when ordered (with appropriate completed/demonstrated skills competency).
5. Assisting in ambulation and exercise accoring to the plan of care.
6. Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills co!llpetency).
7. Assisting patient in the self-administration of medication.
8. Doing patient’s laundry, as appropriate.
9. Meeting safety needs of patients and ·using equipment safely and properly (foot stools, side rails, etc.).
10. Reporting on patient’s condition and significant changes to the assigned nurse.
11. A9hering to the Organization’s documentation and care procedures and standards of personal and professional conduct.

POSITION QUALIFICATIONS

1. Meets the training requirements in accordance with State and Federal laws .

JOB LIMITATIONS
The home health aide will not function in any manner viewed as the practice of nursing according to the State’s Nurse Practice Act. Specifically, the home health aide will not administer medicatiqns, take physician’s orders or perform procedures requiring the training, kowledge and skill of a nurse, such as sterile techniques.

JOB DESCRIPTION SUMMARY
The certified occupational therapy assistant contracted through the Organization is responsible to the registered occupational therapist that is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement. The certified occupational therapy assistant performs only those services planned, delegated, and supervised by the registered occupational therapist.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Participates in the ongoing evaluation of patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self care ability, work capacity, etc.) as delegated by the registered occupational therapist. Participates in the ongoing evaluation of the home environment for hazards or barriers to more independent living as delegated by the registered occupational therapist.
2. Participates in teaching new skills or retraining patients in once familiar daily activities that have been lost due to illness or injury, in accordance with organization policy.
3. Maintains appropriate clinical records, clinical notes, and reports to the registered occupational therapist any changes in the patient’s condition. ·
4. Submits clinical documentation in accordance with Organization policy.
5. Follows treatment program and goals for improved patient function as established by the registered occupational therapist. Documents patient’s response to treatment plan and progress toward established goals.
6. Maintains contacUcommunication with other personnel involved in the patient’s care to promote coordinated, efficient care. Documents such communication in accordance with Organization policy.
7. Attends and contributes to inservices, case conferences, and other meetings as required by Organization policy to ernure coordinated and comprehensive plans of care for the patients of the Organization.
8. Identifies patient and family/caregiver needs for other home care services. Consults with the supervising registered occupational therapist and assists with necessary referrals, as appropriate.
9. Participates in instructing patient’s family/caregiver and other Organization health care personnel in patient’s treatment regime as delegated by the registered occupational therapist.
10. Is supervised by the registered occupational therapist no less than every thirty days. Documentation in the clinical record will reflect ongoing communication between the registered occupational therapist and certified occupational therapy assistant, the patient’s condition, the patient’s response to services provided by the assistant, any need to change the plan of care, and patient outcomes.

Patient Care

1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Regularly re-evaluates patient nursing needs.
3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
4. Uses bealth assessment data to determine nursing diagnosis.
5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
6. Initiates appropriate preventive and rehabilitative nursing procedures. Admi·nisters medications and treatments as prescribed by the physician.
7. Counsels the patient and family in meeting nursing and related needs.
